Release v4.0 Health & Social Care Integration (HSCI) Domain Message Specification

This new version of the Health and Social Care Integration (HSCI) pack contains changed to the "Discharge Notifications to Social Care" and "Care & Support Planning Assessments" sub domains.
The changes to "Discharge Notifications to Social Care" relax previous restrictions on the conformance models by relaxing previously mandated classes and data items from mandatory to optional. For example, the EncompassingEnounter class is now relaxed to be optional.
The changes to the "Care & Support Planning Assessments" include an extra document code in the Contact Assessment that allows this document to act as a referral document, as well as the addition of a Referral Accept and Referral Reject message pair.
